Make an Appointment

Appointments can be made by calling 503-838-0045 during business hours.

New patients are encouraged to complete registration forms in advance and bring them to their first appointment. To access forms click the links below, they are also available at the health center or can be mailed with advanced notice.

Forms

If the appointment is for a sports physical, an Oregon Student Activity Association (OSAA) Sports PhysicalExamination-2023 English/Physical Examination Form-2023 Spanish is required and must be signed by a parent/guardian.

Insurance & Payment Information

No student is denied services because of inability to pay. Oregon Health Plan, employer coverage and private insurance plans are billed whenever possible.

Families with no health insurance are provided assistance to see if they qualify for the Oregon Health Plan/Healthy Kids or other insurance programs. Coverage could fully insure children for medical, dental and emergency services.  For more information about the assistance with health insurance click here.

Hours are Monday-Friday, 7:30 a.m.- 4:30 p.m.

The Central Health and Wellness Center is located at 1601 Monmouth Street in Independence, Oregon.

Medical Services

Medical Services are offered by Salem Health qualified medical professionals. The focus is on preventing illness and promoting wellness.  Services include:

  • Health education and wellness promotion
  • Comprehensive physical exams
  • Sports physicals
  • Well child checks
  • Treatment of illness and injury
  • Routine laboratory tests
  • Medications
  • Immunizations
  • Referrals to specialists as needed for services not provided at the center

Dental Services

Capitol Dental provides qualified dental professionals, including a dental hygienist and board-certified dentist. Services include:

  • Dental screenings and assessments
  • Dental cleanings and sealants
  • Fluoride varnish application
  • X-rays and exams
  • Basic restorative (fillings)
  • Extractions (excluding wisdom teeth)
  • Oral health education

Behavioral Health Services

Mental health services are provided by Polk County Behavioral Health and include:

  • Mental health screenings and assessments
  • Mental health family and individual therapy
  • Addiction assessments and screenings (drug, alcohol, and gambling)
  • Addiction counseling (drug, alcohol, and gambling)
